My name is Rebecca and I'm the Registered Manager here at Choices Homecare in Bolton (formerly Mayday Homecare), our service covers both the Bolton and Bury areas and my aim is to always support my team to provide the best care they can, in line with our company Vision (...to deliver a service to be proud of) and our PRIDE Values.


I started with the company a couple of months ago now and am enjoying meeting the staff and service users. I have a wealth of experience in health and social care and am looking forward to continuing to work with my team and growing the service more.

I could sit here for days telling you about what I do daily because no two days are the same.

Working side-by-side with my Care Assistants in the community or the office.
Every day is different and as Senior Care Coordinator, I have to be ready for anything and everything.


Throughout my whole life I was always told 'If you do a job you love, you'll never have to work a single day for the rest of your life' - This has stuck with me throughout and has been my inspiration to pursue my goals in health and social care.


I have 13 years of service now and over this time I have learned so much; the support I have had from all of my colleagues has been really valuable, without their guidance, my day-to-day working life may have been harder.


I also want to take this chance to remind my care staff of all that they do to support someone, once you become a carer. makes a difference, improves lives and helps support individuality, by no means misjudging the value of what you do and the positive influence you can have on someone’s life.


I’m so proud of our Choices Homecare (formally Mayday and part of the Optimo Care Group) family, together we can accomplish so much.

Job Title: Care Coordinator

Hi, My name is Rebecca and I'm a Care Coordinator here at Mayday Homecare.


My responsibilities as a care coordinator include consulting with service users and determining their needs, developing an individualized plan of action that includes coordination from all parties involved in order to provide the best possible patient care. I Develop and coordinate healthcare programs, and maintain excellent communication between staff, patients, family, professional and outside agencies while maintaining service user confidentiality and keeping up quality care.


I have worked for Mayday Homecare 2 years but previously worked for Mayday as a carer back in 2018. I have a very compassionate nature, am knowledgeable about healthcare practices and provide exceptional customer service to assist patients in handling their health conditions.


In the last few weeks I have been nominated for an award in reflection of my commitment to my role as a care coordinator and I am a finalist for the Care Co-ordinator Award within the Great British Care Awards 2023. Alongside work I am a mother of 4 and have juggled my role as well as a busy home life, striving to take on every challenge and opportunity to effectively support the delivery of quality person-centred care service, supporting local people to stay in their own homes for longer.

My responsibilities as a care coordinator include Working with people, their families and carers to improve their understanding of the service users’ condition and support them in developing and reviewing personalised care and support plans to manage their needs and achieve better healthcare outcomes.


I Help people manage their needs by answering queries, making and managing appointments, and ensuring that people have good-quality written or verbal information to help them make choices about their care.


I Work collaboratively with other primary care professionals within the PCN to proactively identify and manage a caseload, which may include service users
with long-term health conditions, and where appropriate, refer back to other health professionals within the PCN.
I Work with people, their families, carers and healthcare team members to encourage effective help-seeking behaviours;
I also Conduct follow-ups on communications from out-of-hospital and in-patient services;
I Maintain records of referrals and interventions to enable monitoring and evaluation of the service.
I support practices to keep care records up-to-date by identifying and updating missing or out-of-date information about the person’s circumstances;


I have worked for Mayday Homecare as a coordinator for the last 12 months but previously worked for Mayday as a carer out in the community for 4 years.


I have a Proven record of excellent written and verbal communication skills and interpersonal skills alongside evidence of excellent knowledge of Microsoft Office. I am able to deal with service users sensitively while using my motivational and influencing skills.
