The team are there to support people with day to day living tasks so they can continue to live in their own home and maintain elements of their independence as much as possible. The team consists of a diverse group of qualified and caring staff who can offer home care services which are based on the person's current support needs. The team are aware of the importance of effective communication with the person receiving support as well as people invested in their wellbeing such as their family members. The team are also familiar with how to communicate effectively with health professionals and the importance of this in ensuring continuity of care.

The management team undertake a thorough assessment process to ensure that staff are aware of how best to offer support. This includes learning about the person's historical support needs, current physical, social and emotional needs, as well as their communication preferences. This information is then shared with members of the team prior to any support being provided.

The organisation currently uses Access Care Planning software which enables members of the team to complete support notes and, where medication is administered by staff, medication records digitally. This allows health information to be shared more easily with family members and health professionals where required. It is also more compliant in managing data security than paper based records.

The organisation's aim is to provide high quality care and support within Halifax and the surrounding area. Support is currently being provided in the villages of Westvale and Barkisland. The management team are reachable by phone, email and post. They can be contacted by phone at any time and are reachable by email during office hours (8:30am-6pm). Whilst these are the most cost effective and quickest ways to be reached, if it is preferred to use post, this will be responded to the same working day.
