COMMUNITY CARE COORDINATOR

Triband Care Services LTD provides home care services throughout Staffordshire & Staffordshire Moorlands. We enable people to enjoy a more independent life within their own home, providing a wide range of care and support. We are recruiting a Community Care Coordinator to work alongside our Practice Manager and other Community Care Coordinators to ensure the effective running of the Community Home Care Team.

Work/Duties:

Our Service Users are individuals and they all deserve to be treated as such. Community care includes providing personal care to our service users in a respectful and dignified way, including bathing, washing, dressing, continence management, issuing medication, meal preparation, maintaining their health and wellbeing, domestic tasks and any other tasks required as part of their care plan.

·  You will be responsible for coordinating the rota, ensuring the rota is full to capacity, liaising with the local authority Stoke-on-Trent City Council & Staffordshire County Council care brokerage to offer care packages we are able to facilitate, arranging assessment of new Service Users - completing initial assessments and any other related paperwork.

·  Managing the Care Planner/Tagtronics, a monitoring system used to monitor service user call time delivery, ensuring calls are completed on time and for the required duration, planning Service User visits and allocating staffing.

·  Attending Service User reviews with family and fellow professionals, implementing changes and updating care plans and relevant paperwork.

·  You will manage the Community Care staff, complete supervisions every 6 weeks, and deal with queries raised.

·  You will complete on-call duties on an ‘On Call’ rota ensuring that any queries or emergencies are dealt with in a timely manner offering advice and guidance, ensuring staff sickness is covered contacting staff not on duty to see if available to come on shift. If you are unable to source cover as an call manager, you would be required to go on shift to ensure the effective running of the service.

Areas:

·  Stoke-on-Trent, Stone, Stafford, Leek, Cheadle, Biddulph & Surrounding Areas.

Transport:

·  Must drive with a full UK driving licence and have access to a car.

Shift Pattern:

·  Full-time / permanent contracted hours available.

·  Office days 09:00 - 17:00.

·  Fully Flexible between 06:00 - 22:00 to ensure availability in the instance of emergency shift cover.

·  Alternate weekends.

What will be required:

·  A minimum 2 years of care experience, 1-year experience within a management role, ability to effectively manage a staff team and work well within a team.

·  Commitment to making a difference in the lives of individuals whom we provide a high standard of care and support for and the ability to provide hands-on personal care in a way that is dignified and respectful.

·  Good written and verbal communication skills.

·  Full driving licence and own car is essential.
