CarePlus Healthcare Services is looking for a care Coordinator to join our team at Woolwich Arsenal office. In this role you will be required to be effectively and efficiently managing the delivery of high quality care services to our clients.
To accept new client referrals and ensure client schedules are matched to their needs and requirements.
To work closely with the recruitment team to ensure sufficient staffing levels are achieved.
To add and mantain all client and care professional information onto the electronic scheduling system and our own software.
Create, update and audit client care plans and assist with digital care planning.
Mantain accurate client and care professional records on careplus Software and People Planner
Carry out reviews and action these accordingly
Work in line with CQC standards and regulations
Carry out any other duties deemed necessary for the successful operation of the business.
Providing personal care to clients during periods of staff absence.
