Health Care Assistant

We're looking for care professionals to be part of our Quick Response Team working in the community across Graves End and  chsthsm area Medway kent. We provide support to service-users to help them to regain the skills of daily living and full independence if possible, also assessing needs for on-going care if required. SGM Care team delivers this service on a daily basis, making a difference to individuals who may have lost their confidence, motivating them to achieve tailor-made outcomes to regain their independence in their own home. This will involve working closely with healthcare professionals in Gravesend, Kent and Chatham Kent
