Due to our continued growth, we are pleased to be recruiting an additional Field Care Supervisor to join our existing team within our CQC outstanding care company. We are looking for someone who can manage the service delivery of all service users ensuring to develop of individual personalised outcome-focused care and support plans and assisting Abbots Care are fulfilling all compliance and contractual obligations.

The role:
- Complete assessments with our service users and develop personalised person-centred care plans
- Complete spot checks to identify any changes in requirements and amend care plans appropriately.
- Audit medication charts regularly highlighting any additional training requirements.
- Complete coaching and mentoring of care assistants in the company.
- Attend new referrals and advise service users on different care options available to them.
- Build and maintain professional relationships with all areas of Abbots Care and external stakeholders
Person Specification:
- Minimum of 2 years of previous domiciliary care experience.
- NVQ level 2 or above in Health and Social Care.
- Ability to complete and maintain records and reports e.g. service user assessments and reviews.
- Holding a full driving license and access to your own vehicle.
- The ideal candidate for this role will be approachable, caring, kind and non-judgmental.
- Be self-motivated and maintain calmness under pressure.
- Experience working in a faced paced environment and being flexible.
- Strong attention to detail and be a completer/finisher.
- A team player with strong interpersonal skills.
- Strong Communication skills.
- Ability to both lead and be part of a team providing direct care and support.
Desirable Experience:
- Experienced as a Field Care Supervisor in a community care setting.
