Job Description:
As a Care Practitioner at By Your Side, you will play a vital role in supporting customers with their daily activities while promoting their independence and well-being. Your duties will include:

Key Responsibilities:

  • Personal Care: Assisting clients with bathing, dressing, grooming, and toileting.
  • Meal Preparation: Preparing nutritious meals and assisting with feeding if required.
  • Medication Support: Administering medication in line with the customer's support plan and ensuring correct dosages are given at the right times.
  • Mobility Assistance: Helping clients with mobility, including the use of hoists, wheelchairs, and other mobility aids.
  • Companionship: Providing emotional support and engaging clients in meaningful activities.
  • Household Tasks: Assisting with light housekeeping duties such as laundry, cleaning, and shopping.
  • Health Monitoring: Observing and reporting any changes in the client’s physical or emotional well-being to the care manager.
  • Documentation: Keeping accurate records of care provided, including daily logs and any incidents or concerns.
  • Liaison with Family Members: Communicating effectively with family members and other professionals involved in the client’s care, ensuring seamless support.

  • Previous experience in a care role (preferred but not essential, as full training is provided).
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Compassionate, patient, and reliable attitude.
  • Willingness to undergo an enhanced DBS check.
  • Ability to work flexible hours, including evenings and weekends if required.
