Care Manager Responsibilities:

• Creating and updating personalised care plans for customers.
• Collaborating with customers' families, friends, and social supports in developing treatment plans.
• Identifying and recruiting high-quality healthcare staff.
• Ensuring customers’ treatment requirements are met.
• Suggesting alternative treatment plans when customers' services requests do not meet medical necessity criteria.
• Instructing and educating customers on procedures, healthcare provider instructions, and referrals.
• Linking customers to social services programs and entitlements such as transportation assistance and translation services.
• Conducting regular follow-ups with customers to evaluate progress, promote continuity of care, and ensure improved health outcomes.
• Allocating visits to care staff.
• Deal with day-to-day office management

Care Manager Requirements:

• 2+ years of work experience in health and social care sector at management level.
• To have the right attitudes 
• Proficient with Healthcare Management Systems and Microsoft Office Word and Excel.
• Outstanding communication skills, both written and verbal.
• Excellent organisational skills.
