You will be part of a team working towards, and meeting these expectations, providing a wholly personalised approach to the delivery of our services.

Reporting to a Care Coordinator, you will work as part of a team delivering care and support to adults, who are living with a wide range of needs in their own homes. You will also be a part of a wider multi-disciplinary team working in partnership with other health and social care professionals. You will be expected to develop and maintain relationships with service users, their family and carers and other professionals. 

RESPONSIBILITIES
The overriding responsibilities of the Community Care Worker is to provide a safe, effective and efficient service, meeting the needs of the service users and achieving the desired outcomes.

•   Personal care and support
•   Assisting with bathing, showering, dressing 
•   Continence care, catheter care, providing support with catheters, stomas and continence pads and assisting service users to access toilet facilities
•   Pressure care support
•   Meal preparation, serving nutritious meals, feeding 
•   Hydration tasks, ensuring adequate fluid intake
•   Domestic support, ensuring that daily household tasks are completed to a standard required by the service user
•   Medication support in advising, assisting and administering
•   Ensuring the safety and well – being of the service user
•   Recording and reporting care visits
•   Ensuring you are following policy and procedures
•   Ensuring you are meeting legal and contractual requirements
 

Skills / Qualifications:

  • Must be a car driver
