We are seeking an experienced Care coordinator to help continue to develop our home care across south wirral. The Care Coordinator will coordinate and carry out annual reviews of client care files and plans, to ensure clients may continue to receive care within their own homes and within the community, and support our team of care coordinators. The role is extremely fast paced and therefore requires someone who can remain calm and positive whilst working under pressure and to deadlines. Based out of our Hooton office, the successful candidate would need a good knowledge of the South Wirral area.

Main Duties and Responsibilities include:

  • Assessment of new clients care needs and development of care plan

  • Reassessments of existing clients and review current care plans

  • Liaise with all family, medical, professional and other resources as required

  • Provide excellent customer service levels to all internal and external users of the business

  • Carry out timely reviews of spot checks, supervisions and appraisals of care staff

  • Provide support to the Care Coordinators where needed

  • Provide on call cover out of hours service cover, working as part of a rotated team

  • Maintain excellent and accurate communication between the workforce and management

Requirements

  • Previous experience as a Care Coordinator, preferably within the Domiciliary Care sector

  • Obtained a Level 3 QCF in Health & Social Care (or equivalent)

  • Hold a current Full UK Driving licence and have own vehicle

  • Be able to work flexibly due to the need for Out of Hours cover

  • Computer literate with experience of Microsoft Office

Other desirables

  • Experience within the Domiciliary care sector

  • Experience of developing and maintaining client care plans to current CQC standards

Part-time hours: 21-35 per week Up to £22,000.00 per year
