Medical Partnerships Care Team

1.  Main role

  • To provide Care in accordance with current best practice, according to policy and procedures, agreed standards, legislative requirements, relevant regulations under the direction of the Manager.
  • To provide support to family, friends and other healthcare professionals to promote collaborative working.
  • To support Management in the effective delivery of excellent quality of care
  • To be kind, understanding, compassionate and caring at all times

2.  Main Duties

Care Services

  • Address as a first point of call any complaints and where necessary to escalate to Management.
  • Assist clients with personal care as required
  • Assist in the assessment of care records when required
  • Be flexible and open to change in the delivery of care
  • Develop effective communication and relationships with clients and other family support networks
  • Encourage clients where possible to promote independence and explore new grounds eg hobbies
  • Ensure that clients rights are protected at all times
  • Identify and Report incidents as soon as they happen eg abuse
  • Maintain confidentiality of clients and staff at all times
  • Prompt medication as required
  • Record relevant activities in the required logs accurately and factually eg Visit Logs, Incident logs
  • Support a work atmosphere which promotes a high quality of work life
  • Support and maintain a culture of performance and excellence
  • Support clients to maintain their independence, integrity and dignity at all times
  • To carry out impromptu or adhoc duties as directed by Registered Manager
  • Work collaboratively with Management and Team Leaders to promote Best Practice
  • Work in accordance to guidance as set in the organisational Health and Safety Policy
  • Work with other carers to share Best Practice

3.  Working within the organisation

  • Assist in the formulation and implementation of Care policies and procedures
  • Develop effective working relationships with other employees within Medical Partnerships
  • Encourage health promotion strategies
  • Encourage innovative methods for the delivery of care
  • Familiarise yourself with the organisational policies
  • Minimise legal risks
  • Participate in the development of Medical Partnership’s policies
  • Participate in the evaluation of goals against agreed organisational goals, business and quality objectives
  • Seek opportunities for personal and professional growth
  • Support an open positive and inclusive working culture
  • Support the effective resolution of team conflicts
  • Systematically solve day to day problematical issues which arise
  • Work in a cost effective manner by minimising waste
  • Work to establish effective employer-employee relationships

4.  Expectations

  • Attend reflective meetings as scheduled
  • Be involved in Service Improvement programmes
  • Ensure that the correct PPE is worn at all times
  • Familiarise yourself with the Staff Handbook located on BrightHR
  • Join in social functions where feasible
  • Timesheets must be submitted by Monday at the latest
  • To attend and or complete schedule training sessions which includes mandatory and non-mandatory courses.
  • To attend Team Meetings

Skills / Qualifications:
