Healthcare Assistant

We're looking for care professionals to be part of our Quick Response Team working in the community across Woolwich, Plumstead and Thamesmead area. We provide support to service-users to help them to regain the skills of daily living and full independence if possible, also assessing needs for on-going care if required. SGM Care team delivers this service on a daily basis, making a difference to individuals who may have lost their confidence, motivating them to achieve tailor-made outcomes to regain their independence in their own home. This will involve working closely with healthcare professionals in Gravesend, Kent and Greenwich Borough
