Bluestar Care & Support are looking for an experienced
Care Coordinator
Your role will be to organise and coordinate all aspects the home,
providing a safe and quality service to the service users in line with
our company policies and procedures. The Care Coordinator will be
responsible for the safety and wellbeing of the Service Users and
support staff.
- Organising and managing a well balanced rota that meets the needs
of the service users and company training requirements at all times,
ensuring daily routines and all activities can be fulfilled as per
each Care Plan.
- Effective Medication Management – to handle and oversee medication
in the safest manner possible.
- Responsible for the safe keeping, monitoring and management of
service users money.
- Responsible for ensuring all records for the home and service
users are completed correctly to the highest standard.
- Supervise and support all staff in the home.
- Undertake general tasks which contribute to the smooth running of
the home and support the needs of the business and service users.
- Ensuring you engage with any training and your CPD.
- Liaise with all departments, services, colleagues, professional
bodies and service user families in order to consistently complete
all duties and tasks to the highest standard, through meetings,
reports or other appropriate means relative to requirements in line
with the Company Policies, Procedures and Practises.
To become a Care Coordinator for Bluestar you should be
- Knowledgeable of the Care industry and working in Domiciliary Care
( Home Care).
- Have minimum of 1 year experience working with Home Care with
Dementia, Older age, End of life support and respite care.
- Have a minimum of 1 years experience of working with adults with
Learning Disabilities and Complex Needs.
- Knowledge of contacting locan authorities, council and social
workers in order to bring new contracts to company.
- Knowledge og- Intial assessments, care planning, risk assessment
and operational activities.
- Knowledge and understanding of Engagement training.
- Good working knowledge & understanding of: Care Act 2014, CQC
key lines of enquiry, Mental Capacity, SOVA, Autism, Medications,
Positive Leadership, Meaning of and importance of maintaining
confidentiality and GDPR.
- NVQ level 5 or equivalent in Health & Social Care or be
working towards it.
- Medication trained with a minimum of 1 years experience, including
reordering, auditing and management of medication.
- Trained in carrying out supervisions, observations and appraisals.
- Requires good time management, excellent organisational skills, a
confident and calm manner and a positive and proactive approach.
- Demonstrate ability to build appropriate professional, friendly
and accessible relationships with other employees, service users,
seniors, professional bodies and families.
- Demonstrate resilience and ability to work in an organisation that
is undergoing change due to development.
- Demonstrate good working ethics with strong sense of responsibility.
- Maintain a professional, positive and motivating attitude
especially during busy or stressful periods.
- Ability to work flexibly (over a 40 hour week to the needs of the
Service users and company.
- Demonstrate an appropriate philosophy of care which is consistent
with the values of the service.
