The role of Care Coordinator includes many different duties. You will directly line manage a small number of support workers and health care assistants within Great Prospects Care Ltd, ensuring that their needs are being met, and that they feel supported during their employment. You will also directly manage the care of allocated clients.

Great Prospects Care Ltd is an independently owned company providing Domiciliary Care, supported living and community support to adults in their own home, who cannot wholly look after themselves in ways that have positive outcomes for the clients and promote their active participation. We provide a range of support to enable clients, or their loved ones, to live independently in the comfort of their home.

This role includes covering approximately two thirds of client visits and one third office work and consultations, ensuring that service reviews, and quality assurance visits are completed. You will also be expected to undertake supervisions with support workers and health care assistants and mentoring checks.

The shifts are usually, 06:00 to 14:00, 09.00 - 17.00 and 14:00 to 22:00 system. The right candidate will take part in our on-call rota. We are a growing company, and this would be an ideal role for candidates with care experience who wants to further develop their skills and further their career in the Health and Social Care sector. The ideal candidate needs to have a proven background in care however full training will be provided to the right candidate.

Roles and responsibilities also include:

  • Make sure there is sufficient staff cover to deliver care and support on time
  • Monitor the allocation of carers to maximise efficiency whilst supporting them through the use of call monitoring systems
  • Work with the office team to maintain up to date client electronic and handwritten records.
  • Being able to work in a fast-paced working environment, with lots going on around you.
  • Ability to resolve any conflicts which may arise from clients, staff and stalk holders.
  • Keen to learn to operate our rostering software.
  • Ability to multi-task, proactive and resourceful
  • Ability to process short-term roster changes as well as plan regular visit schedules of new clients.
  • You will require experience in a similar role with qualifications in Health and Social Care.
  • You should be used to thinking on your feet, juggling priorities, flexible with excellent telephone manner and good computer skills.
  • You must have the right to work in the UK; full driving license and full use of a car, being on DBS update service will be an added advantage.
