Service Delivery Coordinator Job Summary

StellarCare NW is currently seeking a compassionate, experienced, and a highly-organized care coordinator to join our growing team. In this role, you will play an important part in our ability to provide exceptional care by managing the individual care providers, including scheduling and providing support for the care support workers and families.

Roles and Responsibilities:

  • Communicate effectively with care support workers, clients, families, and other parties
  • Manage care support worker schedules and requests for time off
  • Manage all client schedules, ensuring care plans are being met
  • Maintain accurate client documentation
  • Participate in/conduct care support worker training and induction
  • Maintain compliance with all legal and company requirements at all times
  • Ensure that our clients are receiving the very best in care
  • Willingly participate in night/weekend on-call rotation
  • Support care support workers, clients, and families by providing accurate information
  • Maintain a compassionate demeanour at all times
  • Remain calm and collected in difficult situations
  • See that all measures are taken to protect client privacy and dignity
  • Document and report any issues or violations to upper management immediately
  • See that client care charts/logs are maintained and monitor client progress
  • Work to foresee challenges and implement proactive solutions
  • Demonstrate ability to be a problem solver
  • Provide information to prospective clients and follow up with them
  • Participate in interviewing, hiring, evaluation, and discipline processes
  • Providing care and doing rota for care staff.

Job Types: Full-time, Permanent

Salary: £22.000 to £24.000

  • Driving licence (Required)

Skills / Qualifications:

  • NVQ level 2 and or 3 or working towards
  • Experience in healthcare
  • and/or domiciliary care preferred
  • Minimum 2 years’ experience as a care coordinator or other relevant role
  • Experience with the system Access Care Planning
  • Solid computer skills
  • including Microsoft Office
  • Familiarity with office equipment and phone systems
  • Exceptional communication skills
  • both written and verbal
  • Superior organizational and interpersonal skills
  • Able to diffuse conflicts and disagreements
  • then find a workable solution
  • Able to act decisively and maintain discretion
