Key Responsibilities:

• Provide personal care and support to clients in their homes.
• Assist with daily living activities, including dressing, bathing and meal preparation.
• Offer companionship and emotional support.
• Administer medications as prescribed.
• Maintain accurate records of care provided.
• Follow care plans and communicate any concerns to the care management team.

What We're Looking For:
