Essential Criteria

  • Registered Nurse with active NMC registration.

  • A dedicated commitment to delivering exceptional, client-centered care

  • Genuine compassion and empathy in all care interactions

  • A warm, approachable demeanor with strong interpersonal skills

  • Physical capability to provide direct, hands-on personal care

  • Strong collaborative skills with the ability to work effectively in team environments

  • Full UK driving license and access to a car.

  • Eligible to work in the UK with a clean DBS and two professional references.

Case Management:

  • Conduct in-depth evaluations of service users with complex support needs

  • Design, implement and review personalized support plans

  • Coordinate multi-agency teams to deliver integrated care

  • Facilitate communication between service users, families and care providers

  • Track outcomes and adapt support strategies as required

  • Manage transitions and support successful community reintegration

Service User and Family Education:

  • Provide guidance to service users and carers on managing complex conditions

  • Train on proper use of assistive technologies and home care methods

  • Develop tailored educational resources to meet individual needs

Professional Development:

  • Lead training programs for staff on complex care approaches

  • Maintain up-to-date knowledge of best practices and sector developments
