Essential Criteria
Registered Nurse with active NMC registration.
A dedicated commitment to delivering exceptional, client-centered care
Genuine compassion and empathy in all care interactions

Physical capability to provide direct, hands-on personal care
Strong collaborative skills with the ability to work effectively in team environments
Full UK driving license and access to a car.
Eligible to work in the UK with a clean DBS and two professional references.
Case Management:
Conduct in-depth evaluations of service users with complex support needs
Design, implement and review personalized support plans
Coordinate multi-agency teams to deliver integrated care
Facilitate communication between service users, families and care providers
Track outcomes and adapt support strategies as required
Manage transitions and support successful community reintegration
Service User and Family Education:
Provide guidance to service users and carers on managing complex conditions
Train on proper use of assistive technologies and home care methods
Develop tailored educational resources to meet individual needs
Professional Development:
Lead training programs for staff on complex care approaches
Maintain up-to-date knowledge of best practices and sector developments
Enhance training materials to reflect current standards and innovations
