Northwich - Social Prescribing Community Walk

Last week, a small group of us met up in Northwich for a leisurely walk around the park for a chat. This was arranged by the Social Prescribing Team in Northwich.

Social Prescribers or "Well-Being Coaches" as they like to be referred as, are professionals who work in the community to support individuals by connecting them with non-medical services and activities that can improve their well-being. These services, also take pressure off the NHS!

Rather than focusing on clinical treatments alone, social prescribers help individuals access resources that can address broader social, mental, and emotional needs. These services might include things like local exercise classes, arts programs, community groups, housing support, or financial advice.

The role of a social prescriber is to work with people in a more holistic way, understanding their individual circumstances, and helping them navigate available resources in their community. This can often involve working closely with GP surgeries, mental health services, local authorities, and voluntary organisations. A Well-being Coach can improve a person's quality of life by connecting them with the right social and community support services, addressing the broader determinants of health such as isolation, housing, or access to activities that promote mental and physical well-being.
