We only discovered the extent of Mum's decline in June when she came to stay with us. Following a lot of guilt, upset and research we found the manager at Transitions, she was very understanding and a care plan was implemented for Mum's return home giving us peace of mind as I am a wheelchair user and we live an hour and a half away from Mum. The girls that visit Mum are great and she loves them all, they go above and beyond their remit when Mum has problems and in my mum's words 'Oh we have a right laugh'. Mum was found in the garden quite confused by a neighbour, the neighbour called Transitions and one of the girls called in straight away even though it was her day off. In a nutshell, we would be lost without the care and diligence of all at Transitions.
I struggled to find the right home care company for my elderly dad - poor quality care, terrible timekeeping, inconsistent untrained staff. Until I contacted Transitions! Dad could not be happier, excellent timekeeping, good quality care, helpful, consistent, compassionate staff. My dad could not be happier with Transitions!
Everything is spot on. Carers and management are organised and reliability is perfect. Cannot thank Transitions Care enough. You have made our lives secure and happy.
We were recently on holiday in Hambleton and felt so lucky to have the manager and her lovely colleagues help care for my mother-in-law. I can not thank them enough for the care they provided and am so grateful we managed to get a holiday which was down to the manager managing her staff going above and beyond to help us out as we were struggling to get two carers four times per day. Your hard work dedication and kindness every day were so much appreciated. Thanks again ladies, you all are one in a million wish there was a lot more caring people like you all.
I am extremely happy with transitions care. There is real continuity I only have two carers all the time and they are both lovely. Always on time and very good at what they do they always consider my dignity and respect myself and my home, they also go above and beyond and I appreciate everything they do for me.
I have always found Transitions Care carers to be kind, attentive and compassionate with mum under sometimes difficult circumstances. Their attitude has been professional with thoughtful responsive attention to mum's needs. They have always been cooperative and helpful in my dealings with them.
I moved to Transitions back in 2019 when I was unhappy with other Care Teams who'd been looking after my 87 year old mother. Whereas theirs was a 'tick-box' approach to delivering care, I was impressed by Transition's approach. Transitions had a genuine commitment to customer care. Each client was treated differently, with time spent understanding, respecting and catering to their needs. There was one exception who did not seem to buy into this ethos. And every dealing I had with this person was problematic. She has recently succeeded to the post of Manager - and most of the people who worked with my Mum and looked after her home have now left. Perhaps you should be aware of this if you are basing your decision about whether to engage with Transitions upon earlier reviews on this site. Indeed, it might be wise to insist on a face-to-face meeting with the Manager before committing.
The team of three carers have been friendly and caring to Mum. As there are only three who visit her, she doesn’t feel overwhelmed by different people every day and has quickly got to know and accept them. This has been a relief to the family as Mum was resisting having care given to her - even though she really needed it. The carers are with her for the whole hour - no travelling time included in that hour. They are very keen to recognise and act on her needs and also accept suggestions from the family as to how else they might assist her. The file detailing Mum's care from them is kept at Mum's, for both staff and family to access, meaning we are kept informed of every aid given to Mum, but also reassuring us that all info re Mum's care/needs are noted by staff and steps taken to help her and her safety have been taken. The carers have only been with Mum for a few weeks, but we have already noticed an improvement in her physical and mental health.
Transitions are without doubt, the best service provider I have used. They are professional, conscientious and reliable. All the staff are trustworthy and responsible. Communication is excellent. I would not hesitate to recommend this company to anyone.

And he communicated this to his team who were empowered to be flexible and use initiative for the good of their clients. But this Manager left in December and his replacement has few of his people skills, compassion or empathy. The mood has changed. Good staff members are leaving and long-standing clients like my Mum have been fired because Transitions can “no longer meet their needs”. (We were given only a month’s notice. I live in London and was finding it hard to select a replacement Care provider. I pleaded for a few days’ extra care until I was able to get home and look after Mum myself. The request was refused.) Which was typical of a regime where “going the extra mile” has been replaced by "never give an inch". Be warned: Transitions is going through its own transition.
We received first-class support for my daughter, arranged at short notice for a holiday weekend with the family. Transitions Care Fylde accommodated our late arrival and adapted to our needs throughout the weekend. My daughter was at ease with her new carer, who was always very professional, cheerful and friendly.
I continue to be delighted with Transitions Care. My relative has been supported by them for nearly 5 years and we have seen many changes in that time- not least of which has been Covid. The professional and caring way that Transitions have supported him throughout this time, has been excellent. He is much younger than most of the people they support and they are constantly looking for ways to improve his quality of life. Swimming and birthday celebrations have been recent examples of their caring approach. He is included in all discussions about his support, as am I, which I think is great, we really feel that we could not ask for more. He is about to move to a new flat and we are receiving so much help with this from them. I was concerned as to how we would manage, but needn’t have worried. We will continue with Transitions, knowing that we are in very safe hands.
Following a spell in hospital having had a stroke, my mother required additional support in the home. This was to enable her to regain confidence, independence and the capacity to have the necessary help with everyday hygiene, personal care and mobility. The whole care plan was discussed with both Mum and myself to ensure that her needs were met. Times were arranged which were convenient. Mum felt right from the beginning that, despite her age, she was being listened to and what she wanted was being provided. The staff were introduced to Mum to alleviate the issue of someone strange knocking on her door which would have caused more anxiety. Gradually, different carers took Mum outside for walks and eventually to the shops where once again Mum was able to take control of her independence. A massive 'thank you' to all concerned for the patience, care and support provided during those weeks following her return home.

I cannot express strongly enough what a difference Transitions make in my parents' lives. The care is individual and respectful. They willingly help my parents with whatever challenges arise, going above and beyond in every way. They look at things in a holistic way, but it is always clear that my dad is their client and it is about what is right for him. As I don't live locally, knowing that Transitions are there every day brings a sense of security. The main carer is thoughtful and practical, an all-round great carer.

I was pleasantly surprised to find the levels of knowledge and care, which were issued and the patience and willingness which were demonstrated to sort out irregular and unusual situations. I just want to express my sincere gratitude to Transition Care Fylde. My lovely ninety five year old mum has now gone into a care home but I managed to keep her in her own much loved home for as long as possible. One of Transitions' aims is to match carer with the client as far as possible and they achieved this splendidly. They also provide the same carer for five of the seven visits which matter a great deal. help with washing, shaving, showering and dressing is given sensitively, along with encouragement towards continued independence. Time for conversation afterwards is generously given, which is particularly valuable an appreciated as a help towards restoring sociability. Kindness, concern and respect feature largely in Transitions car of clients and offers and provision of help make life seem more secure.
When I first got with Transitions I was very sceptical about the service they provided because it wasn't an ordinary care company. I had very low self-esteem and being with Transitions they have included me in team meetings. Gone to college to get some certificates. They have helped me change my house quite a lot. It was very bare when Ic same into my house but with some decor, we have changed it from a house to my home. I cannot recommend Transitions more so.
Transitions Care ensure my care needs are completely met with no fuss from all participants. Small changes in daily care routines are arranged freely without problems e.g. whether to have a bath or not, whether to have my hair washed or not. Far more care facilities are available than I wish or need to take advantage of from Transitions Care.
